Forms Online Solutions provides document preparation assistance for U.S. passport application forms. We are a private service and are not affiliated with the United States Government or any government agency.

2. What We Provide

Our service includes: completing passport application forms based on information you provide, reviewing forms for completeness, and providing personalized instructions for submitting your application to the appropriate government agency.

3. What We Do Not Provide

We do not process or submit passport applications. We do not guarantee approval of any application. Government fees are separate from our service fees and must be paid directly to the U.S. Department of State.

4. Payment

Our service fee is charged upon submission of your application information. This fee covers document preparation and is separate from any government fees. All payments are processed securely.

5. Refunds

Refund requests must be submitted within 24 hours of payment, provided processing has not begun. Once our team has begun preparing your forms, refunds are evaluated on a case-by-case basis. Contact contact@formsonlinesolutions.com for refund requests.

6. Accuracy of Information

You are responsible for providing accurate information. We are not liable for delays or rejections caused by incorrect information provided by you.
